It covers the referral form, contact enquiries, newsletter subscriptions, and general website usage. Obligations relating to clinical confidentiality, health records, mandatory reporting, and therapeutic services are addressed in our Client Consent Form, provided to all clients prior to commencing services.

2. Definitions

In this Policy:
4. “Personal Information” is information that may identify you or by which your identity might be reasonably determined. This may include your name, address, email address and phone number. It does not include “Sensitive Information”.

5. “Sensitive Information” means any information about an individual’s racial or ethnic origin, political opinions, religious beliefs, philosophical beliefs, trade union membership, sexual preference or practices, criminal record, or health information.

3. Collection of Your Data

6. We may collect Personal Information and Sensitive Information (together, ‘your Data’) that you enter onto our website, provide to us directly, or give to us in other forms.

7. Your Data collected through this website may include:

8. Sensitive Information, including health information, will not be collected unless you
consent to the collection of that information.

9. Referral Submissions. Our website provides an online referral form through which parents, caregivers, and other health or education professionals may refer a child. By submitting a referral, the person making the referral confirms that:

• they have authority to provide the personal and health information of the child being
referred;

• where applicable, they have obtained the consent of the child’s parent or legal

• the information provided is accurate and complete to the best of their knowledge.

Information collected through the referral form may include the child’s name, date of birth, address, legal guardian status, relationship of the referrer to the child, reason for referral, and funding type (including Medicare/Mental Health Care Plan details). This information is used solely to assess and progress the referral and to provide our services. We do not collect Medicare card numbers or government identifiers through this form.

10. Children’s Privacy. We collect information about children only where necessary to provide our paediatric services. Children’s information is handled with appropriate sensitivity, stored securely, and is not used for marketing purposes.
